Assign observations within the person’s scope

Equipment checks follow the relevant instructions, risk assessment and competent responsibilities. The operational record should say which asset and assigned task it covers. Staff need a clear way to record work they cannot complete rather than making “not checked” appear to be a pass. Technical inspection, testing and servicing keep their authorised people and source records.

Describe the observation without guessing its cause

A useful fault report identifies the asset, visible condition, time, immediate response and person informed. Attach evidence only where safe and permitted. For example, a damaged accessible lead can be recorded with its asset reference and withdrawal action; the reporter does not need to diagnose an electrical fault. Keep the original observation distinguishable from the later technical assessment.

Keep the restriction visible until authorised review

Assign a next action and review point, then record the competent outcome under the approved process. A completed inspection round may correctly leave equipment unavailable. Review repeated findings by asset and delayed actions as well as submission counts. The later repair should remain connected to the original failure rather than changing that failure into a historical pass.
